Proactive Thrombosis Prevention: Patient and Health Professional Forum

NATF’s Annual Forum focused on the improvement of patient care through advancement of thrombosis education. The Proactive Thrombosis Prevention Forum provided a unique educational experience, allowing patients and their families to interact with the professionals who carry out research, provide clinical care, or work on public policy or advocacy issues related to thrombosis.

The North American Thrombosis Forum (NATF) is a 501(c)3 nonprofit organization that focuses on unmet needs and issues related to thrombosis and cardiovascular diseases such as deep vein thrombosis, pulmonary embolism, myocardial infarction, peripheral arterial occlusive disease, and stroke.

Target Audience
This program is intended for physicians, nurses, physician assistants, pharmacists, hospitalists, and hospital administrators interested in thrombosis treatment and prevention.

Disclosure Policy
Med-IQ requires any person in a position to control the content of an educational activity to disclose all relevant financial relationships with any commercial interest. The ACCME defines “relevant financial relationships” as those in any amount occurring within the past 12 months, including those of a spouse/life partner, that could create a conflict of interest (COI). Individuals who refuse to disclose will not be permitted to contribute to this CME activity in any way. Med-IQ has policies in place that will identify and resolve COIs prior to this educational activity. Med-IQ also requires faculty to disclose discussions of investigational products or unlabeled/unapproved uses of drugs or devices regulated by the US Food and Drug Administration.

Accreditation/Designation Statements

For Physicians:
Med-IQ is accredited by the Accreditation Council for Continuing Medical Education to provide continuing medical education for physicians.
Med-IQ designates this activity for a maximum of 6.5 AMA PRA Category 1 Credits™.  Physicians should only claim credit commensurate with the extent of their participation in the activity.

For Nurses:
Med-IQ is accredited by the California Board of Registered Nursing to provide continuing education to nurses.
Provider approved by the California Board of Registered Nursing, Provider Number CEP 14745, for 6.5 contact hours.

For Pharmacists:
Med-IQ is accredited by the Accreditation Council for Pharmacy Education as a provider of continuing pharmacy education.
6.5 contact hours (0.65 CEUs) of credit for pharmacists. ACPE #476-999-09-001-L01-P.  This program is designed for all pharmacists.
